Flu Shot Clinics for all interested students, faculty and staff.
When: Wednesday, November 5, 8:00am-5:00pm
When: Wednesday, November 12, 8:00am-5:00pm
When: Wednesday, November 19, 8:00am-5:00pm
How: Walk-in patients will be accepted only during the Wednesday
clinics as noted above. Students may call 426-1459 for an appointment
to receive the vaccine at other times. Departments, clubs, etc., that
wish to receive the vaccine as a group are asked to please call Mary
Kenworthy, LPN, at 426-1490 prior to walking in to ensure that adequate
staff are available to administer the vaccines.
*Students and employees must bring ID.
Fee: $20 for students; $25 for faculty/staff.
Students enrolled in the Student Health Insurance Plan (SHIP) will not
be charged this fee. SHIP will be billed. Other insurance plans may
cover the vaccine (check with your plan). Departments that purchase
vaccines for staff members should forward a roster to Mary Kenworthy,
LPN, at MARYKENWORTHY@boisestate.edu, or by fax at 426-3005.
